I was taught and use as my adage, respect the office/rank if you can't respect the man.
I may not like some of my supervisors but I address accordingly with respect.
I may not like you personally but you will get at least the courtesy of the title.
I personally place more value in respecting the man in the rank than when people make you respect them because of their rank.

I agree Pff. It's silly because of the context. If this was someplace official (not that it isn't important) where you were speaking to some sort of official, you would likely say President Bush or Ambassador Pug or Captain Pfff or whichever the appropriate title was and would use it when speaking of that person to another.
But when I'm talking to a friend in a casual setting I'll be talking about Bush and W and Bubba Clinton (an unfortunate phrase I picked up on Lettermen years ago) and we talk about Kennedy and Lincoln and Washington or Nixon, Carter and Reagan and it is acceptable and appropriate.
